טיפולי אנזימים פרוטאוליטים (PE) החלו להיות פופולריים בגרמניה בשנות ה- 60 לטיפול בזיהומים, מחלת מפרקים ניוונית, מחלות המערכת החיסונית, וזיהומים וירליים. המוצרים מכילים בדרך כלל תערובת של pancreatin, פפאין, bromelain,  trypsin ו- chymotrypsin. מחקרים שבוצעו במבחנה ובבעלי חיים הראו ש- PE הם בעלי תכונות המווסתות את המערכת החיסונית ומחסלות תאי גידול. סוברים כי תכונות אלו באות כתוצאה מדגרדציה של immune complexes חריגים. נטילת PE דרך הפה ע"י מתנדבים בריאים גרמה לשחרור של ROS) reactive oxygen species) ע"י polymorphonuclear leukocytes. נתונים ממחקרים אפידמיולוגיים מרמזים על השפעות יעילות של PE כטיפול משלים עבור חולי סרטן הראש והצוואר, מיאלומה נפוצה, סרטן השד, ו- סרטן צוואר הרחם. אך יש מידע סותר מניסוי אקראי של מטופלים אשר עברו הקרנות של אגן הירכיים. נדרשים מחקרים נוספים על מנת לאמת את התועלות של PE. תגובות שליליות מדווחות כוללות הפרעות קלות עד בינוניות במערכת העיכול.

גזע האננס הוא מקור טוב ל- Bromelain. מקור הפפאין - הצמח והפירות של פפאיה.

מנגנון פעולה של אנזימים פרוטאוליטים

למרות שהמנגנון המדויק אינו ברור, PE נחשבים כמפעילים השפעות המווסתות את המערכת החיסונית ע"י הגברת השחרור של ROS) reactive oxygen species) ע"י polymorphonuclear leukocytes או ע"י ייצור TNF ואינטרלויקינים IL-6 ו- IL-1B הגורמים להשפעות רעילות. Bromelain הפגין יכולת להפחית את היכולות הגידוליות והגרורתיות של תאי סרקומה L-1. הוא גם בלם הגירה ופלישה של תאי גליומה ע"י גרימת התבטאות integrins, הדרושים לתהליכים אלה. ניסוי שנערך בעכברים עם מלנומה B16 הראה בלימה של גרורות כאשר נחשפו ל- PE ע"י התבטאות מופחתת של מולקולות CD44 ו- CD54.

במחקר זה השתתפו 649 חולות ב- סרטן השד אשר עברו טיפולים (הקרנות, כימותרפיה או טיפול הורמונלי) ב- 216 מרכזים. מתוכן, 239 משתתפות קיבלו אנזים דרך הפה שהורכב מפפאין, trypsin ו- chymotrypsin, בעוד 410 משתתפות קיבלו רק טיפול קונבנציונלי. התוצאות הראו שתסמינים כגון זיהומים, מחלות עור, כאבים בגידול, כאבי ראש ותשישות כרונית הקשורה בסרטן ובטיפול בסרטן היו פחותים באופן משמעותי בקרב המשתתפות אשר קיבלו אנזים. עם זאת, בגלל המשך הקצר של התצפיות, הן לא חד משמעיות. נדרשים מחקרים אקראיים ומבוקרים על מנת לאשר ממצאים אלה.

100 חולים ב- סרטן הראש והצוואר אשר קיבלו טיפולי הקרנה התחלקו לקבוצות באופן אקראי לצורך מחקר זה. קבוצת הניסוי קיבלה אנזים דרך הפה המורכב מפפאין, trypsin ו- chymotrypsin, בעוד קבוצת הביקורת לא קיבלה את האנזים או פלאצבו. החוקרים גילו ירידה מובהקת סטטיסטית של תסמינים כמו mucositis, קשיים בבליעה, ובעיות בעור שהיו קשורות לטיפולי ההקרנה. בנוסף, מספר המשתתפים שהתקדמו לכיוון של תגובות בינוניות וחמורות היה קטן יותר בקבוצת הניסוי לעומת קבוצת הביקורת. כעת מתבצע ניסוי עם מדגם גדול יותר על מנת לאשר את הממצאים הנ"ל.

56 משתתפים עם אינדיקציה להקרנות של אגן הירכיים לאחר ניתוח התחלקו באופן אקראי לקבוצות וקיבלו אנזים דרך הפה או פלאצבו. המשתתפים קיבלו 4 קפסולות, 3 פעמים ביום, 3 ימים לפני ההקרנה וסיימו ביום האחרון של הטיפול. כל המשתתפים קיבלו אותה כמות של הקרנות. התוצאות לא הראו הבדלים בהפחתת התגובות הרעילות להקרנות, כגון שלשולים, בחילות או הקאות.
